Affiliate Instructions

The affilliate form allows research groups to request University of Utah accounts on behalf of collaborators. University of Utah accounts are used for authentication on Center for High Performance Computing systems, so affiliate accounts are necessary for CHPC users who are not actively employed or enrolled at the University of Utah.

Affiliate form options

The affiliate form has several options: Request Affiliate, Deactivate Affiliate, Extend Affiliate Access, and View Request Status. For new affiliate IDs or to renew expired affiliate uNIDs, use the "Request Affiliate" option. Please note that if you include the existing uNID on the form, the process of providing access is usually faster. If an affiliate account has not yet expired, please use the "Extend Affiliate" option.

Note about required fields on the affiliate form

Not all fields on the affiliate form are necessary. Necessary fields are outlined in red. Notably, a user's Social Security Number is not required.

Previously Issued ID

If the user previously had—or currently has—a University of Utah account (uNID), please enter it in this field. It must be entered as a number; if the user's uNID is u0123456, enter 00123456 (with a leading zero in place of the "u").

First Name and Last Name

Enter the user's first and last name in the respective input boxes. The user's full name must be accurate for identity verification.

Birthday

Enter the user's date of birth. This must be accurate for identity verification.

Affiliate Type

The choice of Affiliate Type will depend on the group requesting an affiliate account.

University of Utah departments and groups

If you are a faculty member or administrative staff in a University of Utah department, enter a type that is relevant to the user. This is likely 10060 U Affiliate.

Other groups (Utah institutions of higher education and select others)

If you are not a faculty member or administrative staff at the University of Utah, please select 10020 CHPC as the type.

Start Date

The Start Date is the date on which the user's affiliate role begins. By default, this is the current date.

Exit Date

The Exit Date is the date on which the user's affiliate role expires. By default, this is six months from the current date, but it may be extended up to one year from the current date. Affiliate accounts cannot be created without an Exit Date; they must be renewed if continued access is necessary.

Authorizing Department

The choice of Authorizing Department will depend on the group requesting an affiliate account.

University of Utah departments and groups

If you are a faculty member or administrative staff in a University of Utah department, enter a department that is relevant to the user. This is likely the academic department the user is working with. Affiliate renewal notices will be sent to departmental administrative staff.

Other groups (Utah institutions of higher education and select others)

If you are not a faculty member or administrative staff at the University of Utah, please select 00640 CHPC. Ctr High Perf Computing as the department.

Reason for Entry/Comments

Enter any comments about the affiliate request in this field.

Notify

Enter the user's email in this field. Notifications about approvals, which include instructions for logging in and changing the default password, will be sent to this email.

Notes on obtaining University of Utah accounts

Please allow one business day after the start date of listed on the affiliate form, or overnight if the start date has already passed, for the uNID to become active.

Passwords are created through Campus Information Services (CIS). Please go to CIS and use the uNID as the username with a lowercase "u" replacing the first zero. The initial password will be the birth date in the format MMDDYY with no dashes, slashes, or spaces. If successful, a notice about an expired password will appear. Click the link and create a permanent password. The username will not change.

Affiliate uNIDs expire and expired uNIDs can be reactivated. The authorizing department gets a list of upcoming expiring affiliates weekly. For University of Utah-affiliated groups, expiration notices are sent to departmental staff. For groups not affiliated with the University of Utah, expiration notices are sent to the CHPC. We check for recent account activity, and if the account is actively being used, we renew. Otherwise, we let the uNID expire.

The CHPC cannot reset the password of your University of Utah account (uNID).
